Welcome From the Nurses at Hardin - Jefferson ISD

In order that we might better assist your child with his/her medical needs at their school, we would like to make you aware of a few things.

·We ask that you please call the school nurse or the office to report your child's absence before 10:00. Please inform us of the reason for the absence, so that we may keep current on what may be going around.

· We also ask that you please make sure your student turns in the emergency card. This card is very important for your child. We will use it to contact you in case of emergency or illness. Please keep it updated throughout the year (example: any phone number changes, address changes, work number changes, etc.) Do not forget to fill out both sides of the card. Please list all health problems (migraines, history of seizures, diabetes, etc), glasses worn, allergies, regular medication taken at home or school, etc. The more information we have the better off your child will be. Also, if actual emergency care is needed, this card accompanies the student to the emergency room or health care facility.

· A medication form must be filled out completely, signed and sent with any medication before it can be given at school, (prescription or non-prescription). Medications will not be given without a medication form filled out and the medication sent from home. Prescription medications must be in the student's name and in the original bottle with the correct information of dosage and times. When getting a prescription filled, ask for a school bottle. This will keep from having to send the medication back & forth on a daily basis. Any long-term medication will also need an additional form filled out by your physician.
